US Passport Service

Reading Public Library, Main Library, is a passport acceptance facility. Passport services are available by appointment only during the following hours:

Monday, Tuesday: 9:30am-6pm
Wednesday, Thursday, Friday: 9:30am-4:30pm
Saturday: 9:30-3pm

Applicants must bring proof of U.S. citizenship, proof of identity; a passport photo; and two forms of payment for the fees. One check or money order is made out to the Department of State. The application fee paid to the Department of State varies depending on the type of service requested.  An execution fee of $35 per application is payable to Reading Public Library, which can be made by cash, Money Order, Visa or Mastercard.

Passport photo services are available at the time of your appointment at Reading Public library for a $15 fee. We do not offer stand-alone photo services. Please indicate if you need a photo taken when booking an appointment.

All applicants, including children, must be present.

All minors must have both parents present, or have a notarized Statement of Consent form (DS-3053) from the absent parent. Your child’s passport must be renewed in person, using form DS-11. Passports for children under age 16 are only valid for 5 years.

Children ages 16-17 must apply for a passport in person with one parent or guardian present. Your child’s passport must be renewed in person, using form DS-11. Passports for applicants 16 and older are valid for 10 years.

Both the child applying and the parent or guardian with them will need appropriate ID. (See section “Identification”)

Passport Requirements Checklist

• Fill Out U.S. Passport Application
Forms are available and must be completed prior to your appointment using medium to heavy black ink and no use of white-out.  Do not sign until an agent asks for your signature.

• Show Proof of U.S. Citizenship
Primary Evidence (One of the following):
• Previously issued, undamaged U.S. Passport (may be expired)
• Certified birth certificate issued by the city, county or state, with parent(s)’ full name. If both parents are not listed then the applicant will need to apply for a new birth certificate.
• Consular Report of Birth Abroad or Certification of Birth
• Naturalization Certificate
• Certificate of Citizenship

• Submit Proof of Identity and a Copy of Proof of Identity on 8 ½” x 11” Paper
Primary Evidence (One of the following):
• Previously issued, undamaged U.S. passport
• Naturalization Certificate
• Valid Driver’s License
• Current Government ID (city, state or federal)
• Current Military ID (military and dependents)

For children ages 16-17 who do not have a valid driver’s license, please bring one of the following:
• A learner’s permit
• A School ID
• A State ID
• Current Government ID (city, state or federal)
• Current Military ID (military and dependents)

Both the child applying and the parent or guardian with them will need appropriate ID.

• Submit One Color Passport Photo, 2×2 inches in size
Passport photo services are available during your appointment for a $15 fee, or you can bring your own. You will need one 2″x2″ photo:
• Taken within the past 6 months, showing current appearance
• Full face, front view with a plain white or off-white background
• Between 1” and 1-3/8” from the bottom of the chin to the top of the head
• Taken in normal street attire; camouflage and uniforms are not permitted

• Pay Passport Fees
To submit a passport application, you will need to pay both an Application Fee (to Department of State) and a Library fee. This will require two separate checks.
Application Fee: Check or money order payable to the Department of State
• Adult (age 16+): Passport Book is $130, Passport Card* is $30
• Minor (ages 15 and under): Child Passport Book is $100, Child Passport Card* is $15
• *Passport cards are not for travel by air. Valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only.

Library Fees: payable to Reading Public Library
• Cash, Check, Money Order, Debit, or Credit accepted
• A $35 execution fee/per person is required for applicants using form DS-11
• $15 is charged per person for photos taken at the appointment

Library fees for all applicants can be combined into one payment.

Payment to the library and payment to the government must be paid separately.

The status of your application should be available online 2 weeks after applying. Visit www.travel.state.gov or call the National Passport Information Center 1-877-487-2778 / 1-888-874-7793 (TDD/TTY) for more information regarding your completed passport application or if you have a travel emergency.

If you received a letter in the mail from the State Department with a request for more information, please review the letter and contact the listed phone number with any additional questions. Due to privacy the library cannot contact the State Department on your behalf.
